Online Forms

Online forms allow users to submit and electronically transfer non-sensitive information to the University personnel. Personally identifiable information like SSN, DOB or financial information like bank account numbers and routing numbers cannot be transmitted via online forms. Web Services can complete the initial setup process for online forms. Webmasters can then use the LDP forms feature to develop individual form fields, setup email notification responses and thank you messages.

Legacy Form App Legacy Forms Application

Legacy online forms are setup and developed by USA Web Services. Upon user form submission an email is automatically generated and sent to appropriate university personnel. Users are then forwarded to a thank you page.

LDP Form App LDP Forms Application  

LDP online forms are developed by the individual webmasters. Upon user form submission an email could be generated and sent to appropriate university personnel. Webmasters will also have access to complete data collection by accessing the “Form Submissions” feature under LDP Forms. After form is submitted a thank you message is displayed on the screen.

Google Forms App Google Forms Application

Google forms can also be used by Webmasters to develop online forms. Google forms can be projected on a page via the use of iframes and therefore are not automatically responsive.

PDF Form Submissions PDF Form Submissions

Webmasters may create, update or maintain PDF forms that can submit data online. Webmasters can do this at their own risk. However Web Services does not create, maintain nor provide support for any submittable PDF forms.
